The ACORD 1 Property Loss Notice is used to report personal or business pipe losses due to fire, theft, lightning, wind, hail, flooding, vandalism, return of water channels, etc. that occur under landlord policies, apartment fires, mobile homes, floods, commercial real estate or inland navigation. The ACORD 2 Automobile Loss Notice is used to report business or personal losses arising from liability to other parties, either bodily injury or property damage or damage to your insured`s vehicle such as the whole, collision, fire, theft, vandalism, towing, rent refund, PIP, etc. ACORD 3 – Notice of Occurrence / General Liability Claim is used for the reporting of liability claims in the field of private or professional customers, with the exception of claims related to the vehicle. CAPSA 4 – Workers` Compensation – First Injury or Illness Report is the only CAPWA Notice of Loss form that you, the officer, are unlikely to complete. This claim is usually completed by the employer and sent directly to the insurance company. This can be an insurance company, an outside administrator, or the state`s workers` compensation agency.

The Wisconsin Employer`s First Employer Injury or Illness Report 4WI – Wisconsin Employer`s First is the appropriate form that the employer must complete if it is subject to Wisconsin`s workers` compensation laws.
